I haven't been following this thread to closely, and I use gphoto2 rather
than gphoto, but the only way I could get it to work with my Kodak DC3400
is as follows:

1- usbcore and usb-ohci must be loaded (I think that usb-ohci is
   specific to the USB controller hardware on your system, for some people
   usb-uhci or another similar module may be required instead).

2- usbdevfs must be mounted

3- the usb module for my camera must NOT be loaded (gphoto2 talks directly
   to the camera, if the module is loaded, it can't get access). Everytime
   I plug my camera in, the module is automatically loaded, so I have to
   manually unload it before I can access the camera.

4- I have to run gphoto2 as root

Perhaps this will give you some ideas for things to try.
