Are you looking at all for CLI auditing (ie record every router command
typed) or just some sort of soft comments ("darn this cisco interface")?

If it is the former, then cisco aaa will log every router command input
somewhere (in addition to authentication and per-command authorization). 
We once had a web-based tool which would let you look at every CLI command
by device, user, etc.  Am not sure, but think that the open source tacacs
package will support this functionality.

If you are looking for instead some sort of note tracking, then ida know :(
