Broken mail servers will do that.  Fortunately, you don't see it much:
Such sites are unable to receive mail from most domains since often
mail servers don't have the same DNS and rDNS.  My site is an example
of that, so is the Beaverton School District, Stream International
(when I worked there), Nike...you get the idea.  Doing so makes your
email universe very, very small.
