I have something like this for a Windows app that you could modify for
Hylafax. It's got most of the exchanges for Portland. 

I also have a Hylafax server at work with MS clients. They use Winprint
HylaFAX to send faxes. It works nicely. The user just inputs the
telephone number. There are a number of faxing clients, if you
interested...
