Linux Device Drivers by Greg Kroah-Hartmann.

A fine thing in dead-tree form, also available online for free.
My signed copy has gone missing, dammit.

There's also some good documentation in the kernel source tree itself
under linux/docs (I think), and at kernelnewbies.org.
