I am a fan of Roundup: http://roundup.sourceforge.net/ It's pretty
simple by default (it also has the benefit of being able to be run
stand-alone in kind of a demo mode).
As others mentioned, there is also RT, and we use it PSU, but it's only
so likeable: It has very particular dependencies on dozens of Perl
modules, which always drives me up the wall.
