A shot in the dark: what is the value in your driftfile?

/etc/ntp.conf entry "driftfile" gives the location, on Centos it's
/var/lib/ntp/drift

This is the ntp offset in parts per million (i.e. usec per second) and
if it got corrupted or changed it could cause your sw clock to drift
away from real time.
