[PLUG] Some windows launch with title bar and left edge hidden

Running Ubuntu 16.04 with Unity.  Some applications launch with the title
bar under the bar at the top and the left edge under the launcher.  The
applications behaving this way include Firefox and terminal.  There may be
others.  I can use ALT drag to move the window to get to the hidden stuff,
but this annoying.  What controls this, and how can I fix it?
